Pneumatosis Cystoides Intestinalis

Abstract
|
|
|
This is the report of two cases of pneumatosis cystoides intestinalis diagnosed postoperatively at the Presbyterian Medical Center in Taegu, Korea.
This disease is frequently found in the hogs, but it is rare in human population. There are about
300 cases reported to date and there is no generally accepted theory on it¢¥s etiology and pathogenesis. In these two cases, the air-filled cysts were found in the walls of the terminal ileum and the preoperative diagnosis was not possible in both cases.
Case 1 was a 37 year old Korean farmer whos routine chest film showed free air in the right subphrenic space, but these were no other signs or symptoms of gastrointestinal perforation and the possibility of pneumatosis cystoides intestinalis was raised preoperatively.
Case 2 was a 25 year old housewife who had intractable fecal fistula and the resected ileal segment showed numerous small airfilled cysts in the submucosal layer.
The histopathological diagnosis was confirmed on both of these cases.
The reports show that about 55% of the cases had coexisting stenotic type ulcer either in stomach or duodenum and in only 15% of the cases pneumatosis was the only lesion found. In our two cases, there were coexisting duodenal ulcer, bowel obstruction or fecal fistula.
Both patients had uneventful recovery following the resection of the terminal ileum affected by the gas cysts.
